Duragesic®  (fentanyl transdermal) 50 mg/h

1 patch, every 72 hrs Sharma An opioid originally prescribed May 8, '01, inceased to 2 patches Jun 19, '01. Nurse "Cathy" reduced back to July 2, '01. Neila chose to stop this entirely a week later, but then nurse Cathy recomended going back to 2 a day after a fall on July 12th, 2001. On July 17th increased to 75 mg/h. We removed it July 25. With the crisis of Oct 28th, the hospice nurse started 75 mg/h.
Ecotrin® (Enteric Asprin) 325 mg 1 daily over-the-counter Predates onset of cancer. Low-dosage aspirin recommended by Dr. John Wood
Oxy IR® (Oxycodone) 5 mg 1 or 2 every 3 to 4 hours Sharma For "break-out" pain in conjunction with Duragesic®
Paxil® (Paroxetine) 20 mg 1 daily Sharma An anti-depressant, originally prescribed in 1995 by Dr. Jivesh Sharma. She has taken Paxil continuosulsy since. Dr. Perkins increased this to 40 mg daily starting Oct 1999, but reduced dose back to 20 mg daily in April , 2000.
Prochlorperazine (Compazine®) 10 mg 1 every 6 hours as needed Perkins Started in April 2000 for nausea associated with chemotherapy. Continues as needed use for nausea.
Valium®, diazepam 2mg as needed Perkins Predates onset of cancer. Takes rarely.
Zantac® (Ranitidine) 75 mg 2 daily over-the-counter Originally prescribed 1996 by Dr. McIntyre in 150 mg dose, continues with the over-the-counter 75 mg.
